On the HCFA-1500 form, Box 33a is used to report the Billing Provider’s National Provider Identifier (NPI). This 10-digit, federally assigned number uniquely identifies the billing provider in HIPAA-compliant electronic transactions.

Tip: Always enter the provider’s NPI in Box 33a. This is a required field for electronic claims. Do not report legacy identifiers here—those belong in Box 33b if specifically required by the payer.

In our example below, the billing provider reported NPI 1234567893.

In the X12 837P 5010 transaction, Box 33a maps to the 2010AA Billing Provider Loop, specifically:

  • NM108 – Identification Code Qualifier (XX = NPI)
  • NM109 – Billing Provider NPI (e.g., 1234567893)

Note: Box 33a is mandatory for all HIPAA-covered transactions. The NPI reported here must match the provider registered with the payer to ensure proper claim processing and payment.
