On the HCFA-1500 form, Box 32b is used to report the Service Facility Secondary (legacy) Identification when required by a payer. This is the companion to Box 32a (NPI). Only populate Box 32b if the payer explicitly requests a non-NPI identifier for the service facility.

Tip: Most payers accept the service facility NPI in Box 32a and do not require a legacy ID in Box 32b. If a legacy ID is required, include the correct qualifier followed by the ID number.

In our example below, the facility reported qualifier G2 with ID 54321.

HCFA-1500 Box 32b - Service Facility Secondary ID

In the X12 837P 5010 transaction, Box 32b maps to the 2310C Service Facility Location Loop in the REF segment:

  • REF01 – Reference Identification Qualifier
  • REF02 – Service Facility Secondary Identification Number

Common REF01 Qualifiers for Box 32b

  • 0B – State License Number
  • G2 – Provider Commercial Number
  • LU – Location Number

Note: Use Box 32b only when a payer requests a specific legacy identifier for the service facility. Otherwise, report the facility’s NPI in Box 32a and leave Box 32b blank.
