HCFA-1500 Box 32a - NPI

On the HCFA-1500 form, Box 32a is used to report the Service Facility National Provider Identifier (NPI). This is a 10-digit, federally assigned number that uniquely identifies the service facility location where care was rendered.

Tip: Enter only the facility’s NPI in Box 32a. Do not use legacy identifiers in this field—those belong in Box 32b, if required by the payer.

In our example below, the service facility reported NPI 1234567893.

In the X12 837P 5010 transaction, Box 32a maps to the 2310C Service Facility Location Loop, specifically:

  • NM108 – ID Qualifier (XX = NPI)
  • NM109 – Service Facility NPI (e.g., 1234567893)

Note: Always populate Box 32a with a valid 10-digit NPI if the service location is different from the billing provider’s address. This ensures the claim reflects the actual location where services were performed.
