HCFA-1500 Box 24f - Charges

On the HCFA-1500 form, Box 24F is labeled “Charges” and is used to report the charge amount for each service line. Enter the dollar amount for the procedure reported in Box 24D for that same line.

Tip: Enter amounts in dollars and cents with two decimals (e.g., 150.00). Do not include a currency symbol. Each service line must have its own charge.

In our example below, the provider reported a charge of $150.00 for the service line.

HCFA-1500 Box 24F - Charges

In the X12 837P 5010 transaction, the Box 24F amount maps to SV102 in the 2400 Service Line Loop.

Segment Breakdown (focus on charge):

  • SV101: Procedure code (+ optional modifiers), e.g., HC:98940:25
  • SV102: Line item charge amount (e.g., 150)
  • SV103: Unit/Basis for Measurement Code (e.g., UN = Unit)
  • SV104: Units/Quantity (e.g., 1)
  • SV107: Composite Diagnosis Code Pointer (e.g., 1)

Note: Ensure the charge in Box 24F/SV102 aligns with payer fee schedules, contract terms, and any applicable modifiers that may affect pricing for the service line. Reported charges may differ from the amount expected to be paid. The payer will adjudicate and determine the allowed amount based on the contract or fee schedule.
