HCFA-1500 Box 24d - Procedures, Services, or Supplies

On the HCFA-1500 form, Box 24D is labeled “Procedures, Services, or Supplies” and is used to report the CPT/HCPCS procedure code along with any applicable modifiers. The procedure code is required, while modifiers are optional depending on the service being billed.

Tip: Always ensure that the procedure code entered is valid for the date of service. Include modifiers only when necessary to clarify or adjust the service being reported.

In our example below, the provider reported procedure code 98940 with modifier 25.

HCFA-1500 Box 24D - Procedures, Services, or Supplies

In the X12 837P 5010 transaction, Box 24D data maps to the following:

  • SV101 – Procedure code and optional modifier(s) at the service line level (2400 Service Line Loop).

Segment Breakdown:

  • SV101-1: Product/Service ID Qualifier (HC = HCPCS/CPT Code)
  • SV101-2: Procedure Code (98940 = Chiropractic Manipulative Treatment)
  • SV101-3: Modifier (25 = Significant, separately identifiable E/M service) – Optional

Note: The procedure code is required. Modifiers should only be included when applicable to explain or adjust the billed service.
