HCFA-1500 Box 24b - Place of Service

On the HCFA-1500 form, Box 24B is used to report the place of service (POS) code for each line item. This two-digit code indicates where the service was rendered, such as in a physician’s office, hospital, or other healthcare setting.

Tip: Always use the correct two-digit POS code as defined by CMS and payers. The most common code is 11, which represents a physician’s office.

In our example below, the provider reported a place of service code of 11 (Office) for a single service line.

HCFA-1500 Box 24B - Place of Service

In the X12 837P 5010 transaction, Box 24B data is captured in two places:

  • CLM05-1 – The overall claim-level place of service (2300 Claim Information Loop). This is the typical spot for POS.
  • SV105 – The service line–level place of service (2400 Service Line Loop) only when the line’s POS differs from CLM05-1.

Segment Breakdown:

  • CLM05-1: Claim-level Place of Service (e.g., 11 = Office)
  • SV105: Line-level Place of Service, used only when different from CLM05-1

Note: Populate SV105 only when a specific service line occurred in a different setting than the claim-level POS in CLM05-1. If they are the same, omit SV105.
