On the HCFA-1500 form, Box 24A is used to report the dates of service for each procedure, service, or supply billed on the claim. Each service line can include a single date or a range of dates, depending on how the services were performed.

Tip: Dates must be entered in the MM DD YY format on the paper claim form. In electronic claims (837 transactions), dates are transmitted without spaces or separators, using the CCYYMMDD format.

In our example below, the provider reported services on 05 07 25 thru 05 07 25 and 05 08 25 thru 05 08 25.

HCFA-1500 Box 24A - Dates of Service

In the X12 837P 5010 transaction, Box 24A maps to the DTP segment within the 2400 Service Line Loop. The qualifier 472 is used to indicate the service date, with D8 (single date, most common).

Segment Breakdown:

  • DTP01: Date/Time Qualifier (472 = Service Date)
  • DTP02: Date Format Qualifier (D8 = Single Date; RD8 = Date Range)
  • DTP03: The actual date of service (20250507, 20250508)

Note: Each service line requires its own DTP segment. Always match dates of service in Box 24A with the CPT/HCPCS procedure codes reported in Box 24D.
