The PRV (Provider Information) segment in the 2420A loop of the X12 837 (4010) professional claim is used to convey the rendering (performing) provider’s specialty or taxonomy code. This information identifies the specialty of the provider who actually performed the line-level service being billed.

The PRV segment is situational. It should be reported when the payer requires the rendering provider’s taxonomy code at the service line level. Payers may use this information to validate network status, confirm benefit coverage, or apply specialty-specific claim adjudication rules.

In the example PRV*PE*ZZ*111N00000X~:

  • PE – Provider Code indicating the Rendering (Performing) Provider.
  • ZZ – Reference Identification Qualifier, meaning the following value is a Health Care Provider Taxonomy Code.
  • 111N00000X – The taxonomy code for a Chiropractor.
